Information for Patients

Welcome to the PRx Program!

The fruits and vegetables you’ve been prescribed will help manage your cardiovascular disease, Type 2 diabetes, or prediabetes. We recognize prepping, cooking and eating produce can be a challenge for a number of reasons.

 Check out the resources below for support.

Patients are enrolled directly through their healthcare providers. Ask your provider if they participate in the PRx program.

Please note that delivery is typically only available from June-October.

 

SHOPPING SITES

Vouchers are able to be used at our many participating farmers markets, farm stands and grocery stores. 

Things to know, as we carve out new pathways for this program:

  • No change is given for vouchers, please get your purchase as close to $5 increments as possible. 
  • Price Chopper limits use to two $5 vouchers per customer, per day with AdvantEDGE card.

Other Programs to Help Stretch Your Produce Budget
FreshConnect Checks: These checks are given to EBT users who make a transaction at participating farmers markets and vendors. For every $5 you spend on produce, the vendor gives you a Fresh Check worth $2. This multiplies your spending power at market by 40%.

Double Up Food Bucks (DUFB): This program matches the value of SNAP dollars spent on locally grown fruits and vegetables, up to $20 a day. SNAP Users can sign up for the program easily & quickly at participating locations.

Farmers’ Market Nutrition Programs (FMNP/SFMNP): If you are enrolled in WIC, or if you are a low-income senior (60+), you may be eligible for FMNP! Eligible New Yorkers can receive several $4 coupons to buy fresh, locally grown fruits and vegetables at participating locations.

The Nutrition Outreach and Education Program (NOEP) can help you apply for SNAP: NOEP offers one-on-one help applying for SNAP. They can tell if you may be eligible, help you gather documents you need, and help you with your SNAP application. The service is free and confidential.
